Further to our meeting on the 23rd October 2006 at the above address.
As I stated at the meeting I would be requesting any notes or medical record compiled by you in relation to myself.
I understood from our discussions that in principal you had no objections to furnishing me a copy of these notes or report which will or have been compiled by you in relation to me.
I am now formally requesting this under the Data Protection Act 1998. I am exercising my right to access such data directly pertaining to me as the data subject under English Law.
As a Senior Health Professional, I am sure you are aware and understand the right to access medical records that a client has.
I look forward to hearing from you in due course.
